Minor Application

Players under 16 years of age are not allowed to participate in American MilSim Operations, unless approved by the AMS staff.

 

Amerian MilSim does recognize that some players under the age of 16 have maturity beyond their age and does not discourage family teams. This being said all minors under the age of 16 or family teams with children under the age of 16 must fill out the following information and submit it for AMS staff approval prior to any AMS event.

 

Email the following applications / required forms to info@americanmilsim.com

Attached you will find the required paper work for minors to fill out along with the above information. These forms must be NOTARIZED, if you fail to follow these instructions you will not be allowed to enter the AO and will NOT be refunded.

JP, if we signed the minor release forms for RDGXI do we need to redo them for End State?

On the general information about the OP it says if your under 18 this application is for under 16 what if your 17?

Fill out the application, if you will not have a guardian with you at the AO then fill out all the attached documents.

For a teammate. It says the forms are to be notarized. Do you just bring them in for signup the day of the game?

My boys came to Broken Home with their friend's dad last year and I do not remember having to email an application in prior to the event to get permission for them to come.  We just filled out the temporary guardianship, minor medical treatment, and information sheets and they brought them to registration the first day of Broken Home.  My older son is almost 18 and my younger son will be 16 in July.  I am planning to fill out the same forms this year and have them notarized then send them the first day of registration.  They also attended the airsoft even this past March in Roosevelt so they have previous experience at these type of events.  What else do I need to do to make sure they will be allowed to participate?  Thanks in advance for your help.

If the minor has a guardian at the AO and is over 15 then no paper work need to be filled out, so they boys should be good to go.

Not if you will be on site with him, just fill out the father son form.

I am 15 years old almost 16 but I have been playing airsoft for a several years (5 years) I have knowledgeable of airsoft and have been to Ops before. I am not like most younger airsofters I am more serious about the sport and i have been to fields like Ballahack I will fill the application out i just wanted some sort of chance that iay be able to go. Also a buddy of mine wants to go to OP: Blue/Grey and is 15 aswell we are on a small team but a team at that we have gone to ops together and really want to go to this one because we feel it will push us to our Maximum Physical Capabilities. And improve our skills

They will need to be notarized and required when you check-in.

I am still confused here. A buddy of mine is 16, buy will not have a guardian present. Is this acceptable? We play at ballahack regularly. Thanks

I understand, have them fill out the MINOR with GUARDIAN form and mail it to INFO@americanmilsim.com. The real issues we run into are minors UNDER the age of 16.

I am 14 and want to go to this event I am a little confused on what I have to do in order to be able to play in this op. I know I fill out all 3 attachments above and once filled out email them to the email above. once after that do I wait for the approval and if approved do I then register for the op ?

I know it is a little confusing, working on making it a bit more "simplistic".

 

You need to be approved first, by filling out the following:

 

Minors with Guardians present at AO but to playing with

 

Minors Legal name

Age

Parents/Guardians name

Mailing address

Phone

Medical Conditions

Emergency Contact

Team affiliation (if any)

Airsoft/MilSim experience

OPs attended

 

and emailing it to info@americanmilsim.com

I'm planning on flying out from Texas to attend Blue/Grey with my team, which is based in Virginia Beach. I'm 15 and will be flying out alone, so I will not have a legal guardian with me at the op. How do I go about getting approval to attend? I assume I will have to transfer guardianship, but will the 'new' guardian have to attend the op as well? Forgive me for my confusion.

Not at all you will need to have the Transfer of Guardianship form filled out notarized and scanned/emailed/faxed to us prior to arrival. The "guardian" will need to be at the AO.
