Please have these filled out PRIOR to check-in, by doing this you will save staff and yourself time.


 


If you are under the age of 18 you MUST have this signed and notarized by your legal guardian. If they will not be on site with you.

So do we still have to sign the medical forms and the other forms that aren't these?

Each player will fill out a "medical card" and have it on their person, these can be downloaded or filled out on site.
