ATTENTION

 

Player and Staff MEDICAL CARDS became an AMS SOP at OBH3 and it was a boon to our EMT staff (Thank Rick!)

 

All players ARE REQUIRED to have their Medical Card filled out and ON THEIR PERSON (in their IFAK or Sleeve POCKET) while on the active AO. 

 

PLAYER MEDICAL CARDS are a REQUIREMENT at all American Milsim events.

 

The following PDF template may be printed and filled out prior to an AMS event, or may be filled out during the event check-in process.

 

I suggest players to print, fill out and laminate their own Medical Cards for hassle free use at future AMS events.

Thanks for this! Think mine was unknowingly sacrificed in post-BH3 washing machine action... Laminating would be a good call.

Useful Idea! Back when I was in CAP we'd have our emergency information in our right cargo pocket whenever we'd do something. People get hurt, that's a fact, we have to be prepared for when they do.

I think that right sleeve pocket, or right shoulder pocket would be best.  Most uniforms have at least a breast or sleeve pocket plus these pockets are more likely to not have a lot coming in and out of them during the game.  I use my cargo pockets A LOT when I'm playing.

If you can't laminate a freezer bag or other zipable bag works just fine.

Hey I've posted in a couple different forum topics but haven't got an answer but I'm a firefighter/EMT and talked to yall at copperhead and was wondering how I was able to become a certified field medic? Just would like to help if something happened, thanks!

You can use packing tape as well.  Cover both side, and then cut it down with an exacto knife or scissors.

 

 

Also known in the Marine Corps as "recruit lamination".  I have a lot of documentation done like this.  Works great.  Especially the stuff I keep on the motorcycle or in my tactical gear. 

 

As for the blood type comments.  Blood Typing is pretty cheap.  30ish dollars I think, and is great information to have about your self.  I've known mine since I was in grade school.
