They continue to use MILES gear because it's in the system and available and blanks are always available.  SIms not so much.  SIMS are as expensive as shooting live ammo and you can't just grab a rifle or pistol off the shelf that fires real ammo and shoot sims though it.  Not to mention the logistics of it and the safety gear usually required for sims.  

IMO we continue to use MILES because of availability cost of the already paid for gear, weapons, and blanks, not because it's any more realistic than any other form of firearms force on force training.

Actually Sim rounds are nearly twice as much as live ammo. The run anywhere from about $.075 to $1.00 per round. Also the Sim rounds don't provide anymore range than your average AEG. As for realism that's all a matter of opinion, and definition. Every training or simulation method is going to have drawbacks. Unless you're willing to shoot live rounds at your training partners nothing is ever going to be 100%.
